Application form Application form and prospectus can be obtained from the Principal, Karunashray School of Nursing and Para Medical Institute, Vill Basaudi, Post KNI Dist Sultanpur or from Karunashray Hospital, Civil Lines, Sultanpur, on payment of Rs 500/-in person or Rs 550/- by M.O. these should be completed by the candidates in her own handwriting and submitted along with the following testimonials on or before 15th May 20......

Attested copies of:-

1. High school certificate showing the date of birth and mark list.
2. Intermediate mark list or its equivalent.
3. Intermediate certificate.
4. Aadhar Card
5. Passbook (Front page copy)
6. Income Certificate, Caste Certificate, Domicile Certificate.
7. Medical Fitness certificate by a registered medical officer.
(Pregnancy Certificate if married)
8. Two self addressed envelope (size 6" x3.5") with Rs. 25/- postage stamp.

(ORIGINAL CERTIFICATES OF THE ABOVE SHOULD BE PRODUCED AT THE TIME OF INTERVIEW)
